AfyaPro Connected Care App is the eHealth application to support people to better control their diabetes and hyper tension condition. It includes educational material, nutritional advice, a logbook for key measures like blood sugar, bloodpressure, insulin dosage and so on. The App links to a call centre that provides support. An unique feature is the direct linkage of the App with the electronic health record of the health facility associated with the call centre. The chat function allows direct and secure communication between the patient and the health professional.
